Histologically, a dermal based, well circumscribed tumor composed of pleomorphic, irregularly arranged, spindled to epithelioid cells with numerous mitotic figures. Atypical fibroxanthoma (AFX) is a rare spindle cell neoplasm predominantly found on the head and neck of elderly individuals with sun‐damaged skin, with no evidence‐based guidelines for their management. Atypical Fibroxanthoma of the Skin With Metastasis ELSON B. HELWIG, MD,' AND DENNIS MAY, LTC, MC, USA? Cutaneous atypical fibroxanthoma (AFX) occurs in elderly persons as a small nodule or ulceronodule in actinically damaged skin of the head and neck area. The vast majority of AFX behave in a benign manner, and metastasis is rare.

Herein a patient with an AFX that metastasized and eventuated in death is described. Atypical fibroxanthoma (AFX) is a rare skin malignancy generally developed in elderly male patients with actinic damage. Due to its low rate of metastasis, it is considered a benign neoplasm [1,2]. Metastatic AFXs occur in fewer than 6% of patients. A case is presented of postirradiation atypical fibroxanthoma of the skin, metastatic to cervical lymph node, in a young female.
A solitary tumour or multiple tumours may occur. Typically, AFX is a red, juicy, dome-shaped nodule that may be bleeding, ulcerated or crusted.

The potential aggressiveness of AFX is controversial. OBSERVATIONS: We describe herein a patient who developed metastatic disease in cervical lymph nodes. Background: Atypical fibroxanthoma (AFX) is a fibrohistiocytic tumor with relatively high local recurrence rates but low metastatic potential. Wide local excision (WLE) and Mohs micrographic surgery (MMS) are common treatments, although no consensus exists regarding optimal therapy. Abstract. Atypical fibroxanthoma (AFX) is an undifferentiated pleomorphic sarcoma that typically has low-grade malignant potential.
However, varying degrees of subcutaneous involvement can lead to a more aggressive course and rare metastases. A case is presented of postirradiation atypical fibroxanthoma of the skin, metastatic to cervical lymph node, in a young female. Although such skin tumors characteristically have a malignant microscopic appearance, and although they occasionally recur following surgical excision, they have previously been noted for their consistently benign clinical behavior.

Atypical fibroxanthoma is a spindle cell tumor most commonly found on actinically damaged skin, primarily the head and neck, of elderly Caucasians with a small potential for metastasis. Atypical fibroxanthoma lesions occasionally extend to the subcutaneous tissue. Note that atypical fibroxanthoma is a dermal-based process while spindle cell squamous cell carcinoma variants (on the differential diagnosis) connect with the epidermis and show signs of keratinization.
